Abstract
This study aims to create a development plan for the selected areas of Barangay Sagayen, Asuncion, Davao del Norte. In this study, the researcher-made questionnaire was utilized in order to gather the relevant data. There is one (1) set of questionnaire used in this study. The set of questionnaire was employed to identify the top problems of the selected Puroks of Sagayen, based on the survey, water system, health center, farm financing, farm-to-market road, waste management program, livelihood program, and education emerged to be the needs which require the most attention and urgent actions. Through the need assessment survey that was performed, using a questionnaire that was verified and approved by the adviser and panel members, the proponents analyzed the result and was able to derive a proposed community development program. The program was centered to areas that need development. On the basis of the results of the Needs Assessment Survey conducted among residents of some Puroks of Sagayen, several problems and issues were identified which need to be given appropriate actions in order to have a sustainable life. Problems were ranked by the residents and were made as to proposed a development plan on the top three needs that needed the most urgent action as perceived by the proponents. Those three are livelihood program, health center and farm financing.
